Our privacy policy reflects the requirements of section 3.3 of the code of conduct guidelines for the Osteopathy Board of Australia (AHPRA).

CONNECT Osteopathy and Performance respects your right to privacy and is committed to safeguarding the privacy of our patients and website visitors. We adhere to the National Privacy Principles established by the Privacy Act 1988 (Cth). This act sets out how we collect and treat your personal information.
“Personal information” is information we hold which is identifiable as being about you.
Collection of personal information
CONNECT Osteopathy and Performance will, from time to time, receive and store personal information you enter onto our website, provided to us directly or given to us in other forms.
You may provide basic information such as your name, phone number, address and email address to enable us to send you information, provide updates and process your service order.
Additionally, we may also collect any other information you provide while interacting with us.
CONNECT Osteopathy and Performance uses Cliniko as an online booking service.
How we collect your personal information
CONNECT Osteopathy and Performance collects personal information from you in a variety of ways, including when you interact with us electronically or in person, when you access our website and when we provide our services to you.
Information collected in paper copies is typed or scanned into the patient’s file. Paper copies of any sensitive or identifying information are confidentially destroyed.
Use of your personal information
CONNECT Osteopathy and Performance may use personal information collected from you to provide you with information and updates.
CONNECT Osteopathy and Performance may contact you by a variety of measures including, but not limited to telephone, email, SMS or mail.
Disclosure of your personal information
We may disclose your personal information to any of our employees and staff.
We may from time to time need to disclose personal information to comply with a legal requirement, such as law, court order, subpoena, in response to a law enforcement agency request or transfer information to another health service provider.
Access to your personal information
You may request details of personal information that we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth).
Should the patient have a query regarding any of the content of their records, this should be put in writing, with copies to the relevant treating Practitioner and Practice Manager. Such queries will be addressed as quickly as possible.
Signed written consent of the patient is required prior to release of any personal details or medical information. A small administrative fee may be payable for the provision of information.
Should the patient feel that there has been a breach of the Australian Privacy Principles; such concern should be addressed in writing to the Practice Manager. All necessary action will then be taken to rectify the issue and a response in writing, detailing actions taken, will be sent to the patient.
Changes to privacy policy
Please be aware that we may change this privacy policy in the future. We may modify this Policy at any time, in our sole discretion and all modifications will be effective immediately upon posting on our website.
